Please print, fill out and sign the following forms and bring them with you on the first day of class.

Location and parking details will be emailed out prior to your first class.
All new participants will be scheduled for an assessment on weeks one, three and six. These assessments are optional but recommended for accountability. A schedule will be emailed out prior to your first class. The person who sheds the most inches gets a free program!
